Blackhawk handheld radio pouch. Most likely would have been private purchase and not military issue. You can see retail examples here.

agreed, BHI, probably late 90s/early 2000s, after the bulk of their production went overseas. If you take a little nail polish remover to the tag you should be able to read it.
